Your username is your report number. Include hyphens. An example of the correct report number format is RFT-01234-A5B6 or RFR-01234-A5B6.
Your password is that you created when filing the report.
If you do not have your report number or password, you cannot check your report.
Do not use this feature to report 9-1-1 type emergencies.
